ABSTRACT

Se presenta un caso clínico de quiste dentígero, asociado a un tercer molar inferior izquierdo retenido, que concurre a la Cátedra de Diagnóstico por Imáge- nes de la Facultad de Odontología de la Universidad de Buenos Aires. El paciente, de 53 años de edad, es de sexo femenino. Se utiliza la tomografía de haz cónico para diagnosticar presuntivamente dicha patolo- gía. Posteriormente, se realiza biopsia para obtener una muestra y ser enviada para realizar los estudios anatomopatológicos, que corroboran el presuntivo diagnóstico. Basado en lo expuesto, se analiza al quis- te dentígero según ubicación, sexo, edad y maxilar; habiendo realizado una revisión de la literatura (AU)


A clinical case of a dentigerous cyst associated with a retained lower left third molar is presented. The 53-year-old patient is female. Cone beam tomography is used to presumptively diagnose said pathology. Subsequently, a biopsy is performed to obtain a sample and be sent to perform pathological studies that corroborate the presumptive diagnosis. Based on the above, the dentigerous cyst is analyzed according to location, sex, age and maxilla; having carried out a review of the literature (AU)

ABSTRACT

Objetivos: Demostrar la utilidad y la facilidad técnica del injerto tibial en defectos óseos orales y maxilofaciales, para tenerlo como un recurso alternativo en la práctica general del cirujano oral y maxilofacial. Caso clínico: Se presenta un caso clínico con el uso de injerto óseo autólogo de tibia para el relleno de un defecto óseo a raíz de una lesión quística. Se realizó la exéresis de la patología quística por medio de un abordaje oral y posteriormente se recolectó hueso medular tibial a través de un abordaje medio al tubérculo anterior de la tibia, para poder colocarlo en el defecto óseo. Conclusión: El injerto de hueso medular de epífisis tibial representa un sitio de recolección de fácil acceso, del que se puede obtener una cantidad de hueso ideal para defectos de pequeño y mediano tamaño de la región maxilofacial, de baja morbilidad y con muy pocas complicaciones post-operatorias, lo que lo convierte en una alternativa para rellenos de cavidades óseas de gran utilidad (AU)


Objective: To demonstrate the utility and technical ease of the tibial graft in oral and maxillofacial bone defects so as to have it as an alternative resource in the general practice of the oral and maxillofacial surgeon. Case report: A clinical case is shown with the use of an autologous tibial bone graft to fill a bone defect as a result of a cystic lesion. The cyst was excised by an oral approach and the medial tibial bone was collected through a middle approach to the anterior tubercle of the tibia, to place it in the bone defect. Conclusion: The tibial epiphysis medullary bone graft represents an easily accessible collection site, from which an ideal amount of bone can be obtained for small and mediumsized defects of the maxillofacial region, with low morbidity and very few post-operative complications, which makes it a useful option for bone cavity filling (AU)

ABSTRACT

Introduction: Dentigerous cyst is an odontogenic cyst lesion surrounding the crown of an unerupted or impacted tooth in the jaw. Objective: The aim of this case report is to describe the treatment planning and follow-up of a dentigerous cyst associated with the impaction of the permanent mandibular right canine tooth in an 11-year-old female. Case report: Radiographic assessment revealed a radiolucent unilocular round-shaped lesion localized in the mandibular symphysis. Enucleation was performed and the affected tooth was removed under general anesthesia. The patient is under follow-up due to orthodontic treatment. The affected area healed without complications. Conclusion: Two and a half years after the enucleation, the cyst had totally disappeared, and no recurrences were observed. Bone remodeling and neoformation were noticed.


Introdução: O cisto dentígero é uma lesão odontogênica ao redor da coroa de um dente não irrompido ou impactado na mandíbula. Objetivo: O objetivo deste relato de caso é descrever o plano do tratamento e o acompanhamento de um cisto dentígero associado à impactação do canino permanente inferior direito em uma criança do gênero feminino de 11 anos de idade. Relato do caso: A avaliação radiográfica revelou lesão unilocular radiolúcida de forma arredondada, localizada na sínfise mandibular. A enucleação foi realizada e o dente afetado foi removido sob anestesia geral. A paciente encontra-se em acompanhamento devido o tratamento ortodôntico. A área afetada curou sem complicações. Conclusão: Dois anos e meio após a enucleação, o cisto desapareceu totalmente e não houve recidivas. Remodelação óssea e neoformação foram observadas.

ABSTRACT

El quiste dentígero, incluido dentro de la clasificación de los quistes odontogénicos por la OMS en el 2017, constituye una patología relevante en el área buco maxilofacial. Si bien su crecimiento es lento y asintomático, cuando alcanza grandes dimensiones, tiene capacidad para provocar destrucción ósea en los maxilares, lo que ocasiona alteraciones estéticas y funcionales. Además, presenta una alta prevalencia, con el quiste odontogénico como el más común, después del quiste radicular inflamatorio. La radiología representa una herramienta fundamental en su diagnóstico y muchas veces su detección constituye un hallazgo radiográfico. Normalmente, se emplean técnicas en 2D como la radiografía periapical, panorámica y oclusal, las cuales cumplen un rol muy importante en su detección y diagnóstico. No obstante, debe considerarse que la similitud del quiste dentígero con otros procesos patológicos, tanto a nivel clínico como radiográfico, muchas veces lleva a un diagnóstico y tratamiento inadecuado. Con este trabajo se pretende evidenciar si el advenimiento de nuevas tecnologías como la resonancia magnética y la tomografía computarizada de haz cónico permiten mejorar la posibilidad del diagnóstico del quiste dentígero y distinguirlo de otras lesiones o patologías con las cuales se realiza el diagnóstico diferencial. The dentigerous cyst, included in the classification of odontogenic cysts according to the WHO year 2017, constitutes a relevant pathology in the maxillofacial buccal area. Although its growth is slow and asymptomatic, when it reaches large dimensions it has the capacity to cause bone destruction in the jaws, causing aesthetic and functional alterations. In addition, it has a high prevalence, being the most common odontogenic cyst, after the inflammatory root cyst. Radiology represents a fundamental tool in its diagnosis and often its detection constitutes a radiographic finding. Normally 2D techniques such as periapical, panoramic and occlusal radiography are used, which play a very important role in their detection and diagnosis. However, it should be considered that the similarity of the dentigerous cyst with other pathological processes, both clinically and radiographically, often leads to inadequate diagnosis and treatment. This work is intended to show whether the advent of new technologies such as magnetic resonance imaging and conical beam computed tomography, allow to improve the possibility of diagnosis of the dentigerous cyst and distinguish it from other lesions or pathologies with which a differential diagnosis is made. ABSTRACT

Introduccion: según la clasificación de la oms de 1992, el quiste dentígero o folicular se clasifica dentro de los quistes de desarrollo odontogénicos. Este se origina en la evolución de un germen dentario. La pieza dentaria, generalmente, se encuentra en íntima relación con la membrana quística, adherida al límite amelocementario. El crecimiento es lento y continuo. Radiográficamente se observa una imagen radiolúcida, definida y unilocular. Afecta con más frecuencia al sexo masulino y la proporción de pacientes de 6 a 7 años con quistes dentígero es de solo 9.1%. Caso clínico: paciente masculino de 6 años concurre a la consulta con su abuela presentando inflamación facial del maxilar superior derecho, asintomática. Luego del examen clínico se le solicita una radiografía panorámica. Por su severidad, se le solicita una tomografía computada y se lo deriva para su intervención quirúrgica y análisis de la muestra. Conclusion: el odontólogo clínico debe realizar la anamnesis e inspección clínica y ante la sospecha de una patología, debe solicitar un estudio radiográfico y/o tomográfico complementario. Se puede observar que durante el recambio dentario, es frecuente la presentación de lesiones patológicas que si no son diagnosticadas a tiempo estas podrían malignizarse y llevar al paciente a grandes resecciones quirúrgicas (AU)


Introduction: according to the who classification of 1992, the dentigerous or follicular cyst is classified within the odontogenic development cysts. This originates in the evolution of a dental germ. The dental piece is usually in close relation with the cystic membrane, adhered to the amelocementary limit. The growth is slow and continuous. Radiographically, a radiolucent, defined and unilocular image is observed. It affects the masculine sex more frequently and the proportion of patients from 6 to 7 years with dentigerous cysts is only 9.1%. Clinical case: a 6-year-old male patient attended the consultation with his grandmother presenting facial inflammation of the right upper jaw, asymptomatic. After the clinical examination, a panoramic radiograph is requested. Due to its severity, a tomography where it is then transferred for its surgical intervention and sample analysis. Conclusion: the clinical dentist must perform the anamnesis and clinical inspection and, if pathology is suspected, he or she must request a complementary radiographic and / or tomographic study. In odontopediatry, during dental replacement, it is common the presentation of pathological lesions that if not diagnosed in time they could be malignant and take the patient to large surgical resections (AU)

El quiste dentígero es definido como una lesión quística que involucra la corona de un diente incluido que está relacionada con la unión amelocementaria. Clínicamente se manifiesta por un aumento del volumen asintomático, siendo generalmente descubierto en un examen radiográfico de rutina o por la investigación de un diente no erupcionado. Radiográficamente presenta una lesión radiolúcida unilocular asociada a un diente incluido. Este estudio presenta un caso clínico de un paciente con quiste dentígero en la región posterior de la mandíbula, tratado quirúrgicamente por medio de enucleación y exodoncia de los dientes involucrados. Paciente de género masculino, de 9 años de edad, que compareció a la Clínica de casoOdontología de la Universidad de Ribeirão Preto (UNAERP), Brasil, con queja de abultamiento inferior izquierdo de la cara y marcada asimetría facial, la radiografía panorámica y tomografía computarizada evidenciaron la presencia de un quiste dentígero extenso. Delante de este cuadro clínico y radiográfico, el plan de tratamiento establecido fue la enucleación del quiste para permitir la erupción de los dientes sucesores permanentes, seguido por la exodoncia de los elementos dentarios 74/75. El tratamiento quirúrgico se mostró rápido y resolutivo evidenciando que la elección del tratamiento debe ser cuidadosamente evaluado para cada caso.


O cisto dentígero é definido como uma lesão cística envolvendo a coroa de um dente incluso, estando este ligado a junção amelocementária. Clinicamente manifesta-se pelo aumento de volume assintomático, sendo geralmente descoberto pelo exame radiográfico de rotina ou pela investigação de dente não erupcionado. Radiograficamente, se apresenta como uma lesão radiolúcida unilocular em torno de um dente incluso. Este estudo apresenta um caso clínico de um paciente com cisto dentígero em região posterior de mandíbula, tratado cirurgicamente por meio de enucleação e exodontia de dentes envolvidos. Paciente do gênero masculino, 9 anos de idade, compareceu a Clínica de Odontopediatria da Universidade de Ribeirão Preto (UNAERP), Brasil, com queixa de abaulamento inferior esquerdo da face, com acentuada assimetria facial, cujo exame radiográfico panorâmico e tomografia computadorizada evidenciaram a presença de amplo cisto dentígero. Diante do quadro clínico e radiográfico, o plano de tratamento instituído foi a enucleação do cisto, para permitir a erupção dos sucessores permanentes, seguido da exodontia dos elementos dentários 74/75. O tratamento cirúrgico se mostrou rápido e resolutivo, evidenciando que a escolha do tratamento deve ser cuidadosamente ponderada para cada caso.


The dentigerous cyst is defined as a cystic lesion involving the crown of an included tooth, which is attached to the cement enamel junction. Clinically it is manifested by asymptomatic increase in volume, and is usually discovered by routine radiographic examination or by investigation of non-erupted tooth. Radiographically, it presents as a unilocular radiolucent lesion around an even tooth. This study presents a clinical case of a patient with a dentigerous cyst in the posterior region of the mandible, treated surgically by enucleation and exodontia of involved teeth. A 9-year-old male patient attended the Pediatric Dentistry Clinic of the University of Ribeirão Preto (UNAERP), Brazil, complaining of lower left bulging of the face, with marked facial asymmetry, whose panoramic radiographic examination and computed tomography evidenced the presence of large dentigerous cyst. In view of the clinical and radiographic findings, the treatment plan was to enucleate the cyst to allow eruption of the permanent successors, followed by the exodontia of the dental elements 74/75. Surgical treatment was fast and resolute, showing that the choice of treatment should be carefully weighted for each case.

Hybrid lesions of the oral cavity are infrequent and share characteristics with a number of other pathologies. both odontomas and dentigerous cysts are of odontogenic origin, but their simultaneous occurrence is rare and scarce. clinical and radiographic examinations are not conclusive, making their identification difficult, while histopathological studies can reveal their defining characteristics. the aim of this report was to describe the radiographic and histomorphological findings of a hybrid lesion formed by a complex odontoma and a dentigerous cyst, affecting the mandible of a 22-year-old man, from Cartagena, Colombia, who had no relevant medical history, and no symptoms or discomfort in the affected area.

ABSTRACT

Los quistes del maxilar constituyen un conjunto muy variado de entidades, entre las que se incluyen los tumores benignos y malignos de origen odontogénico. Existe poca información sobre la prevalencia de esta patología. Afecta con mayor frecuencia al sexo masculino, entre la segunda y la tercera década de la vida. La proporción de pacientes de 6 a 7 años con quistes dentígeros es de solo 9,1%. El quiste dentígero engloba la corona de un diente permanente normal, impactado, no erupcionado. Suele presentar una sintomatología insidiosa y no se descubre hasta que la tumoración afecta a los órganos vecinos. El objetivo es exponer un caso de quiste dentígero. Si bien es de aparición muy poco frecuente, la presentación de este caso sirve para poner al médico pediatra en conocimiento sobre cómo debe actuar ante una tumoración maxilar unilateral en un niño sano. Se remarca la importancia del manejo multidisciplinario de la entidad.


Maxillary cysts are a diverse group of entities that include benign and malignant odontogenic tumors. Information on the prevalence of this disease is limited. It is more common among males, and usually occurs in the second and third decade of life. The proportion of 6 to 7 year old patients with dentigerous cysts is only 9.1%. Dentigerous cysts encompass the crown of a permanent and unerupted impacted teeth. They are usually slow growing asymptomatic lesions that are not discovered until they affect surrounding organs. The aim of this study is to present an unusual case of dentigerous cyst and to inform the pediatrician about the management of a unilateral maxillary tumor in a healthy child, underlining the importance of a multidisciplinary approach of this disease.

El quiste dentígero es una de las patologías óseas que más frecuentes en el maxilar y la mandíbula. Se evidencia radiográficamente como una imagen radiolúcida y unilocular en la mayoría de los casos. Generalmente es detectado en exámenes de rutina o cuando se investiga la presencia de dientes no erupcionados. El quiste dentígero aparece principalmente en las tres primeras décadas de vida, con crecimiento lento y asintomático. Los terceros molares,así como dientes supernumerarios, pueden estar relacionados con su formación, sin embargo, su etiopatología no es totalmente conocida. La descompresión, marsupialización y la enucleación son las principales formas de tratamiento, pero algunos criterios deben ser considerados, como tamaño del quiste, edad del paciente, proximidad con estructuras anatómicas nobles e importancia clínica del diente involucrado. Para que se establezca un correcto diagnóstico y plan de tratamiento es esencial la utilización de exámenes complementarios de calidad, siendo, actualmente, la tomografía computarizada de haz cónico el más completo y preciso método de diagnóstico por imagen empleado en estas lesiones. El objetivo de este estudio fue resaltar, por medio de la presentación de un caso clínico, la importancia de la tomografía computarizada de haz cónico en el diagnóstico y planeamiento quirúrgico de un quiste dentígero asociado a un diente no erupcionado


The dentigerous cyst is one of the most frequently found in the jaws. They present themselves radiographically as radiolucent images and more commonly unilocular. They are usually observed at routine checkups or when investigating the presence of unerupted teeth. The dentigerous cyst occurs mainly in the first three decades of life, their growth is slow and asymptomatic. The third molars, as well as supernumerary teeth may be involved with the formation of a dentigerous cyst, but its pathogenesis is still not fully known. Decompression, marsupialization and enucleation are the main forms of treatment, but some criteria must be considered for the treatment plan, such as cyst size, age, proximity to noble anatomical structures and clinical importance of the tooth involved.In order to establish a correct diagnosis and treatment plan is essential to make use of complementary exams and now a days the cone beam computed tomography is the most complete and accurate method in diagnostic imaging used in these situations. Therefore, the aim of this study was to highlight, through the presentation of a clinical case, the importance of cone beam computed tomography in the diagnosis and treatment planning of a dentigerous cyst

ABSTRACT

In dental practice, impacted third molar teeth are a common finding. The dentists usually remove them if they are associated with some radiographic finding suggestive of a cyst or a tumor or if they cause pain or resorption of the adjacent teeth. It has been found that, in many cases, even the radiographically and clinically asymptomatic impacted mandibular third molars are associated with some pathology. This paper brings into the light two cases with clinically and radiographically normal impacted third molar teeth associated with dentigerous cyst, thus highlighting that the radiographic appearance is not a reliable indicator of the absence or presence of pathology associated with the impacted third molars.

Se presenta un caso interesante de una niña de 13 años que acude a la consulta de Cirugía Maxilofacial por presentar asimetría facial y bucal, además de referir diplopia y obstrucción nasal del lado de la deformidad. Después de realizar exámenes de laboratorio, radiográficos y tomografía axial computadorizada (TAC), se lleva al salón de operaciones para realizar extracción del tercer molar superior izquierdo retenido en posición invertida, enuclear la extensa área radiolúcida a los rayos X y radiopaca a la tomografía y posteriormente se realiza estudio histopatológico. El diagnóstico definitivo corroboró el presuntivo (quiste dentígeno ). La paciente evolucionó de forma excelente; desaparecieron la deformidad facial y los demás síntomas preoperatorios(AU)


An interesting case of a 13 year-old girl comes to Maxillofacial Surgery consultation because she has facial and buccal asymmetry; she also refers diplopia and nasal obstruction on the side of the deformity. After carrying out laboratory exams, radiographic studies and computerized axial tomography (TAC), she was taken to the operating room to receive an extraction of the third left superior molar retained in inverted position, and to have retrieved the extensive rays- X radiolucid and radiopaque area at tomography and later on she underwent a histopathological study. The definitive diagnosis corroborated the presumption of dentition cyst. The patient evolved in an excellent way; her facial deformity and other preoperative symptoms disappeared(AU)

ABSTRACT

The maxillofacial region is affected by a greater number of cysts than any other part of the body. The present study [A descriptive case series] is about the clinical, features i.e. age, sex and site distribution and radiological features of dentigerous cyst performed on 47 biopsy proved cases over a period of two years. This cystic lesion is common in males [60%] as compared to females [40%]. The most common age group is the second decade [48.9%] of life. The mandibular third molar region [57.44%] is the most favorite anatomical site, followed by maxillary canine region [25.53%]. Swelling has been the most common chief complaint i.e. [61.702%]. In 19.14% of patients, dentigerous cyst was a chance radiographic finding. The most common radiological feature is a well circumscribed unilocular radiolucency surrounding the crown of a tooth [96%] modalities were enucleation [83%], marsupialization [11%] and marsupialization followed by enucleation [6%]
